Family/Group Psychotherapy Session Our family and group psychotherapy sessions provide a confidential, supportive environment for improving communication, resolving conflicts, and strengthening relationships. These sessions are designed to help families or groups work through challenges together, build healthier dynamics, and foster mutual understanding. PETER BOURQUE NP No‑Show Fee Policy & Financial Responsibility Disclosure FINANCIAL AGREEMENT This Financial Agreement outlines your responsibilities regarding payment for services provided by Peter Bourque NP LLC. Our goal is to ensure transparency, prevent unexpected charges, and support uninterrupted access to care. Insurance & Billing If you are using insurance, it is your responsibility to: • Confirm your mental health benefits, copayments, deductibles, and coinsurance • Ensure your insurance information remains accurate and up to date • Notify us immediately of any changes to your coverage We will submit claims to your insurance provider on your behalf. However, you are financially responsible for any portion of services not covered by your plan, including copays, deductibles, coinsurance, or services deemed non‑covered. Payment Method on File All clients are required to maintain an active and valid payment method in their client portal. This allows us to process copays, deductibles, no‑show fees, and any outstanding balances promptly. If the payment method on file is invalid, expired, or has insufficient funds, the balance will remain your responsibility. A late fee of $5 per day will be added for each day the invoice remains unpaid until the balance is resolved. Outstanding balances must be paid in full before follow‑up appointments can be scheduled or confirmed. This policy applies only to billing and scheduling and does not limit your rights under HIPAA or New York State patient rights law. Session Fees If you are not using insurance, or if a service is not covered by your plan, you are responsible for the full session fee. Payment is due on the date of service unless otherwise arranged. Cancellation & No‑Show Fees Appointments missed without proper notice are not billable to insurance. Cancellations made with less than 24 hours’ notice will incur a $50 no‑show fee, which will be charged to the payment method on file. If payment is not submitted, the no‑show fee and any applicable late fees will continue to accrue at a rate of $5 per day. Follow‑up appointments may not be scheduled or confirmed until the outstanding balance is paid in full.
